Telefragging

Grant Voelker requested to merge telefrag into master

Adds telefragging, the killing of players and exploding of barrels that happen to be in the way of teleporting players.

Will require proper gibbing effects and for the trace to run from the center of the destination teleporter to the actual teleport position, in front of the teleporter.
